Early Help Assessment (My Family Plan)

If we're giving you Early Help support, we'll spend time with you and listen to the worries you have, and help you think about what help you need. Part of this involves completing the 'My Family Plan', Middlesbrough's Early Help Assessment.

My Family Plan is nothing to be worried about, it's just a conversation to work out how to stop small challenges becoming big challenges.

It'll cover things like:

  • what's going well
  • what you're worried about
  • what changes you've tried to make in the past
  • who offers you help and support
  • how we (the council) can help you

We'll also talk to your children to understand how they're feeling and find out if there's anything they think they might want help with.

We'll help you put a plan together and then set up a Family Plan Review meeting. We'll bring along other services who might be able to help you.
