Hospital to home support

If hospital patients do not get a safe, comfortable discharge, they're more likely to be readmitted within 30 days. That's where we come in. Our Hospital to Home service helps older, vulnerable, and disabled patients who live in Middlesbrough.

Hospital to Home makes sure patients come home to a safe home environment, and can keep living independently after they're discharged from hospital. This gives confidence to our clients, and peace of mind to their relatives and carers.

Hospital to Home can help you with:

  • practical advice and support
  • repairs and adaptations from our handyperson service
  • assistive technology to help with independence
  • disabled adaptations
  • decluttering
  • installation of a key safe
  • picking up clothes to go home
  • moving small items of furniture
  • gas/electric meter top-ups
  • checking and setting up heating
  • arranging fire brigade safety checks
  • food packages and shopping for essentials
  • picking up wheelchairs from the property
